The family of a woman who died last year has sued The Luxe at Wellington Rehab Center, alleging staff allowed her to develop sepsis, malnutrition and dehydration while she lived there.

Deanna MacDonald, daughter of Mary Olsen and personal representative of her estate, filed the lawsuit in Palm Beach County Circuit Court. It names Wellington Rehab and Health Center, LLC, which the complaint describes as the facility’s licensee and owner. Olsen died May 20, 2025.

The complaint, which BocaNewsNow.com obtained, says staff never built an adequate care plan for Olsen and missed warning signs as infections, weight loss and dehydration set in. It alleges the facility did not notify her family or doctors of those changes, did not follow physician orders and did not get her to a hospital in time. The suit also blames inadequate staffing, poor training and weak supervision of employees.

The case is brought under Chapter 400 of Florida law, which sets out the rights of nursing home residents. The family is seeking more than $50,000 in damages and a jury trial. Olsen’s other survivors are listed as Edward Olsen Jr. and Stephanie Brick. Ford, Dean & Rotundo, P.A., of North Miami Beach, represents the family.

These are allegations in a civil filing, and the claims have not been tested in court. The facility had not responded publicly in the source report.
